Welcome to the DPS Employee Health Screen system. As you complete this screening, please take into account whether your symptoms are "new" or "unexpected", or whether the symptoms are consistent with your usual allergies or an underlying health condition.
Any staff diagnosed with COVID-19 or who, in the last 14 days, was in close contact with someone who currently has COVID-19 or someone who had new loss of taste or smell should not go to work and should isolate or quarantine according to public health recommendations. For additional medical questions or guidance, please consult with your medical provider.
As a DPS employee, I understand that I must conduct a daily health screen to assess myself for COVID-related symptoms prior to entering the building. I acknowledge that this screen is a tool designed to raise my awareness of my wellness and that conducting a daily health screen is a condition of reporting to the worksite.
Note: To complete your daily required health attestation using Safe Access, you must use your DPS employee ID to login. Your DPS ID is a 9 digit number that can be found on your DPS employee ID badge, on
your Infor profile (this option is only available on a PC), or at the top of your paystub under “Employee Number”.
